Disaster Service Worker Form (DSW)
The Disaster Service Worker (DSW) form is an essential document for all Community Emergency Response Team (CERT) members, providing them with official registration and important legal protections while volunteering in emergencies. In California, DSW status covers volunteers for injuries or damages that may occur during their assigned duties, ensuring they receive necessary support and benefits. Completing the DSW form not only formalizes a CERT member's commitment to emergency response but also helps our team maintain a well-organized, coordinated response network. This process ensures that all volunteers are recognized, protected, and prepared to serve their community safely and effectively.
The following is the DSW form that is also given on the first night of the CERT Program Series: DSW Form
